Liability Waiver and Release of Claims

PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT.

I, the undersigned, as the parent/legal guardian of the minor student named above (the “Student”), acknowledge and agree as follows:

1. I understand and acknowledge that participation in dance classes and related activities at Ananda School of Dance (the “School”) involves physical movement and the inherent risk of injury, regardless of the level of supervision or care exercised.

2. I voluntarily assume all such risks associated with the Student’s participation, including but not limited to injury, illness, or other medical conditions.

3. I hereby release and forever discharge Ananda School of Dance, its instructors, directors, employees, agents, and volunteers from any and all claims, demands, damages, costs, expenses, and causes of action arising out of or related to any loss, damage, or injury (including death) to the Student or property, whether caused by negligence or otherwise, while participating in any program offered by the School.

4. I certify that the Student is in proper physical condition to participate in dance classes and has disclosed any medical or physical limitations.

5. I agree to indemnify and hold harmless Ananda School of Dance and its affiliates for any claims brought on behalf of the Student or arising out of a breach of this waiver.

This release shall be governed by and construed in accordance with the laws of the Province of Ontario and the federal laws of Canada applicable therein.
